You are obtaining my services in assisting you in obtaining a FAA flight medical certificate.
By signing completed FORM, you are giving Robert J. Gordon, D.O., AME, informed consent to communicate with, and obtain any records needed, from any of your treating physicians or other health care entities, personal contacts or the FAA on your behalf in order to assist you in obtaining a FAA flight medical certificate. I can only advise and assist you in this process. The FAA aeromedical division is the entity that will issue the medical flight certificate therefore I can make no guarantees of the outcome. I will do my best to assist you in your goal.
FEES $400.00 for initial 20-minute phone or in person consultation to review your current problems and as to why you are unable to obtain an FAA flight medical certificate and set up a written plan on how, from my experience, it is best for you to proceed. If you chose to execute the plan unassisted no further fees will be incurred. If you would like my ongoing assistance my fee is: $400.00 per hour, paid in advance, for my ongoing assistances in helping you obtain the flight medical certificate such as, speaking with health care providers/entities, communication with the FAA on your behalf, submission of information to the FAA, contact with the you (the applicant) regarding the case. The ultimate amount of time involved will depend on several factors, the complexity and number of medical problems, amount of records to be obtained and reviewed and the cooperation of medical providers/entities involved, the FAA aeromedical division and yourself. Assist in understanding what the FAA is requesting or new request of information for a different medical issue. Recommend appropriate medical professional specialist to perform FAA requested or suggested evaluations (Cardiologist / Neurologist / Neuropsychologist / psychiatrist, etc.)Receive documents/reports/medical information provided by the applicant and/or those assisting the applicant
AND, most important, review the documents and reports for thoroughness and provide opinion and feedback aimed at optimizing the certification process.
Submit completed packet to the appropriate place at the FAA for the most expedition review. These fees DO NOT INCLUDE, FAA medical certification examination. Also note, HIMS and non- HIMS require separate tasks and incur separate charges.
